Reporting Requirements for Referees

Information for Referees on the North West Youth Alliance
Referees must receive the team sheets from the clubs no later than 15 minutes prior to Kick Off.
You must complete the team sheet with the following information :
1. The match score
2. All cautions/sending off using the correct code (C1 or S1 etc)
3. The tick list on the back of the team sheet must be completed.
4. Any misconduct by either spectators or club officials must be Emailed to the Referees Secretary.
The complete forms can either be scanned and Emailed to the Referees Secretary (providing you retain the originals until the end of the season) or posted to the following address:
